Persistent Headaches: When Is It Time to See a Neurologist?

Almost everyone gets a headache now and then. Khurram Nazir, MD, a board-certified neurologist and a past member of the Emerald Coast Medical Association board, wants patients to understand the difference between an occasional headache and one that needs medical attention.

According to Dr. Nazir, an occasional headache, brought on by dehydration, a long day in the sun, or a poor night of sleep, is not usually a cause for concern. Regular, persistent headaches are a different story. “Having regular persistent headaches is not normal,” Dr. Nazir explains. “If you’re having them, that needs to be addressed.”

The good news is that most persistent headaches are not caused by anything dangerous. Dr. Nazir notes that hormonal and chemical imbalances, which can affect anyone and are especially common in younger patients, are a frequent cause, and effective treatments exist for many of them.

There is a specific set of symptoms, however, that should prompt a faster evaluation. Dr. Nazir describes these as red flag signs: vision changes, trouble with speech, numbness or weakness, and dizziness occurring alongside a headache. When any of these symptoms are present, he recommends further investigation and a specialist evaluation rather than waiting it out.

Beyond the physical discomfort, Dr. Nazir points out that persistent headaches carry a real cost to daily life. They can affect work, parenting, and relationships in ways that are easy to underestimate. “It is a major, not only a physical complaint, but it is a quality of life issue,” he says.

The path forward often starts with a conversation. Treatment for chronic headaches can range from small lifestyle adjustments to targeted medication, and Dr. Nazir emphasizes that care can be tailored to each patient rather than following a single formula.

If you experience headaches on a regular basis, especially alongside any of the red flag symptoms above, start with your primary care physician and ask whether a referral to a neurologist is appropriate. Persistent headaches are common, but they are not something you have to live with.
